Cranberry Assessment, Referral, and Evaluation

 

C.A.R.E. stands for Cranberry Assessment, Referral, and Evaluation. It is a Student Assistance Program aimed at helping students in all grade levels to overcome issues that interfere with their education. The main goals of C.A.R.E. are to help identify students in distress, to intervene, and to get them to help as quickly as possible.

Any student, parent, teacher, or community member who has a concern may contact a member of the C.A.R.E. team for advice or help. The confidentiality of the student and others involved with the referral is always maintained.

C.A.R.E. Referral Form
This form should be completed and returned to the SSC to refer a student to the C.A.R.E. Program.
